The story of a Japanese man in his 40s who developed extensive nodules on his buttocks and pyoderma around the anus after wearing the same panties without washing them for over 10 years was revealed.

Medical staff at Japan’s Autonomous Medical University announced that Mr. A, a 48-year-old Japanese man, visited the hospital’s emergency room due to worsening breathing difficulties. Mr. A had a skin lesion causing bleeding and pus on his buttocks for 4 years. He revealed that the last two winters had been difficult for him due to shortness of breath, and pain in his hip had made it difficult for him to ride his bike. He also said he only wore one pair of panties that hadn’t been washed in over 10 years and that he didn’t bathe regularly, but did shower occasionally to wash off the pus and blood from his buttocks. Instead of washing the pus and other exudates from his buttocks, he put a towel inside his panties and changed them as needed. An examination of the skin on his buttocks at the hospital revealed abscesses, fistulas, and dark, crusted nodules with bloody and pus-filled exudate on both buttocks. As a result, Mr. A was diagnosed with systemic hypertensive heart disease, heart failure, chronic kidney disease, anemia, and extensive chronic perianal pyoderma. Perianal pyoderma is a disease in which bacteria infiltrate the area around the anus, causing inflammation in the overall skin layer from the outermost epidermis to below the hair follicles, and causing bleeding accompanied by pus. In the end, the medical staff excised and removed the fascial skin and subcutaneous tissue on the surface of the patient’s buttocks. Skin grafting was performed and the patient fully recovered after about six months.

The medical staff said, “This is an example that shows that wearing unhygienic underwear and sitting for long periods of time can lead to chronic perianal pyoderma,” adding, “We must be careful because this has the risk of worsening into squamous cell carcinoma.”

This case was published in the journal ‘Clinical Case Reports’.​
